June 5, 2012, The Change Foundation announced the Northumberland Community Partnership as the winner of its 3-million investment in this trail-blazing PATH project to redesign care for seniors.
Health and social care organizations are uniting with seniors and caregivers to identify care transition problems across Central East Ontario. They will then work with a full range of service providers to redesign care and improve experiences.
Patients Canada is among a coalition of providers, patients and family caregivers who are identifying issues and barriers to healthcare delivery. The work includes the redesign of services to improve patients’ experiences and how they move through the healthcare system.
As one of the partners, our role is to ensure that the patient perspective is well-represented and maintained throughout the project.
